    Overview

    Where you live can greatly affect your health. Your zip code and race shouldn’t determine how long you live, but they do. In fact, health has more to do with race and place than doctor visits. The American Heart Association is committed to determining and improving the role that “social determinants of health” play in our long-term risk of heart disease, stroke and other devastating illnesses. Our first ever Social Determinants of Health Conference in Philadelphia will strive to remove barriers to good health by convening key stakeholders to share experiences on programs, policies and strategies aimed at reducing health inequities. Sample topics of discussion include: • Removing barriers preventing communities from adopting healthy living behaviors • Improving access to quality and affordable healthcare for all • Driving the elimination of food deserts • Increasing access to safe environments for physical activity

    The EmPOWERED To Serve Urban Health AcceleratorTM is a four-week intensive training and grant opportunity designed to challenge you to apply innovative, community-based solutions to help solve an urgent urban health problem.

    Step One – Look Around and See the Problem
    Is there a problem in your community in Philadelphia that has an attainable, sustainable solution for it? Once you’ve determined the problem you feel most passionate about solving, it’s time to roll up your sleeves

    Step Two – Apply Online June 25th through July 23rd, 2018

    Step Three – Activate Your Genius

    Our team will roll up our sleeves with you and you’ll have six weeks to build a plan, activate your team and implement your community project, MacGyver style (in other words, with what you have right now. Yes, it’s possible.).

    The top performing teams will be invited to present their projects to a panel of judges on September 24, 2018, as a part of the American Heart Association’s Social Determinants of Health Conference in Philadelphia, Pennsylvania.

    What are the Awards?

    On the evening of September 24th 2018, our top performing candidates will be invited to present their project to a panel of judges and a live audience.

    First Prize- $30,000

    Second Prize- $20,000

    Third Prize-$10,000
